In order to provide the highest quality training possible Central Ohio Health & Safety Training Center, LLC (COHSTC) wishes to make you the student aware of your rights
and responsibilities as a COHSTC student. Should at any time you have questions regarding your rights and responsibilities or any of these administrative policies of
COHSTC please communicate them to your instructor or to our CEO Gary M. Trentman at 614-374-1330 or info@cohstc.com ATTN: Gary M. Trentman.
Henceforth in this agreement Central Ohio Health & Safety Training Center, LLC and its instructors will be referred to as “we, us, our, ours” and you the student shall be
referred to as “you, your, yours, they, them”.
Varied Learning Needs
We recognize that you may require specialized teaching techniques or strategies based on your learning needs. Please notify your Instructor(s) prior to the beginning of
your training or testing. This is to ensure that we are able to meet your needs within reason of the instructor’s capabilities.
Physical Limitations
We recognize that you may possess physical limitations that will prevent you from performing the necessary psychomotor skills in the standard means. If you possess a
physical limitation you need to notify us, so we can make adjustments for you within a reasonable means. Please Note: If you are a person with physical limitations and can
not perform the necessary psychomotor skills properly, even after adjustments, certification may be held and not issued. Every situation will be reviewed by both our
Director and facilitating instructor.
Certification Cards for qualifying students will be issued after payment
You may not participate in our class if remuneration has not been received in full. If by Prior arraignment remuneration is to be paid after completion of the course so named
above, the certification of those who completed the course satisfactory shall be released after payment is received in full. Please Allow 30 Days for Processing.
“Pay Once Policy”
In our mission to continuously provide quality health and safety training we have adopted the “Pay Once” policy. Should a student not be able to pass the certification
process (if applicable) remedial instruction will be provided until successful completion, without further cost. Contact the Client Relations Manager for full details. Subject to
limitations and additional terms and conditions.
Class Paperwork is Proprietary and Confidential
You understand that we own all instructional and testing material. You may purchase through us a course book or course material, if it was not included in the cost of the
class. You recognize that we own all course records, course evaluations, Student rights and responsibilities acknowledgement forms, and contracts. You may request
copies of contacts and acknowledgement forms or others for legitimate business or certification/licensure reasons.
Medical Asepsis/Decontamination of Training Equipment
We adhere to the highest standards of medical asepsis/decontamination when cleaning and decontaminating practice equipment. It is however unlikely, but possible for
disease transmission to occur while using our equipment. To yet further reduce the possibility of disease transmission the student acknowledges that they will follow the
instructor’s directions on proper use of the equipment. We cannot make guarantees for facility provided equipment.
Personal Safety
We cannot guarantee the reliability or safety of the facility, facility grounds or equipment on or by the facility. We do not condone discrimination, violence, assault or an
environment that is threatening in nature. Please contact your instructor if you have concerns. We reserve the right to remove any person from the class for any reason that
interrupts the learning experience or is not providing for the safety of all.
Privacy Policy
You hereby authorize us to contact you via the information you provided for any valid business need, including issuing certifications, offering renewals, advising of near
expiration or expired certifications, collecting debts, invitations to newsletters and other reserved purposes. The full policy is available at http://www.cohstc.

Certification Replacement
If for any reason you need a certification replacement, such as, incorrect name do to illegible writing, loss, theft, destruction, or any other unforeseen reasons the
replacement fee is $10.00. You must return the original card or the remnants thereof; or sign an affidavit that it lost or stolen and pay the fee prior to the replacement being
issued.
Requirements for licensure
For some professionals maintaining certifications is mandatory. If your certification has lapsed or will lapse soon you may request in advance that certifications be
produced on site for $20.00 per certification, or have a Rush placed for $10.00 per certification. Subject to successful completion of the course. If you are using this class as
continuing education credits it your responsibility to ensure that it qualifies and you have the proper paper work.
Mailing Certifications
If you are taking this class as it is offered by an organization or for an organization, the certifications will be mailed to the designated contact person for that class. If you are
in an Open Enrollment class or Fill To Capacity class and are paying for your own certification it will be mailed to the address provided. If you are part of an organization and
want to have the certification mailed to your address, approval in writing must be obtained from the contact, and a $5.00 Processing Fee paid.

recognize that our instructor(s) will never force you to perform an action that you cannot safely perform. You recognize that we do not own, lease or rent the premises and is
NOT responsible for any malfunction or poor condition. You have acknowledged that you cannot hold us responsible for injury. Finally you recognize that if you abuse and/or
damage, or carelessly misuse our equipment and/or materials you will be held responsible for the full cost of the replacement. Should injury result of your misuse/abuse
you will NOT hold us liable. You also acknowledge that you forfeit your right to sue in court and will be bound to independent arbitration after resolution has not been
obtained through our customer service channels
